BackgroundIn cancer care, do not resuscitate orders are common in the terminal phase of the illness, which implies that the responsible physician in advance decides that in case of a cardiac arrest neither basic nor advanced Coronary Pulmonary Rescue should be performed. Swedish regulations prescribe that DNR decisions should be made by the responsible physician, preferably in co-operation with members of the team. If possible, the patient should consent, and significant others should be informed of the decision. Previous studies have (...) shown that physicians and nurses can experience ethical dilemmas in relation to DNR decisions, but knowledge about what ethical reasoning they perform is lacking. Therefore, the aim was to describe and explore what ethical reasoning physicians and nurses apply in relation to DNR-decisions in oncology and hematology care.MethodsA qualitative, descriptive and explorative design was used, based on 287 free-text comments in a study-specific questionnaire, answered by 216 physicians and nurses working in 16 oncology and hematology wards in Sweden. Comments were given by 89 participants.ResultsThe participants applied a situation-based ethical reasoning in relation to DNR-decisions. The reasons given for this were both deontological and utilitarian in kind. Also, expressions of care ethics were found in the material. Universal rules or guidelines were seen as problematic. Concerning the importance of the subject, nurses to a higher extent underlined the importance of discussing DNR-situations, while physicians described DNR-decisions as over-investigated and not such a big issue in their daily work.ConclusionThe study revealed that DNR-decisions in oncology and hematology care gave rise to ethical considerations. Important ethical values described by the participants were to avoid doing harm and to secure a peaceful and “natural” death with dignity for their dying patients. A preference for the expression “allow for natural death” instead of the traditional term “do not resuscitate” was found in the material. (shrink)

Cloud computing (ClC) has become a more popular computer paradigm in the preceding few years. Quality of Service (QoS) is becoming a crucial issue in service alteration because of the rapid growth in the number of cloud services. When evaluating cloud service functioning using several performance measures, the issue becomes more complex and non-trivial. It is therefore quite difficult and crucial for consumers to choose the best cloud service. The user's choices are provided in a quantifiable manner in the current (...) methods for choosing cloud services. Hence, this study attempts to achieve this objective through construction. decision-making framework so-called cloud services evaluator (CSsEv). The main indicator and its sub-indicators are formed as nodes at levels(n) in tree soft sets (TSSs). Thereafter Single Value Neutrosophic Sets (SVNSs) as branch of neutrosophic sets which conjunction with the Multi-Criteria Decision Making (MCDM) technique to facilitate analysis and evaluation process for the available Cloud services providers. Hence, entropy is employed to obtain indicators and sub_indicators weights and Complex Proportional Assessment utilizes these weights to facilitate the decision process of selecting optimal ClSPs. Artificial intelligence (AI) national strategies provide countries with a framework for the development and implementation of AI technologies. Sixty countries worldwide published their AI national strategies. The majority of these countries with more than 70% are developed countries. The approach of AI national strategies differentiates between developed and developing countries in several aspects including scientific research, education, talent development, and ethics. This paper examined AI readiness assessment in a developing country (Palestine) to help develop and identify the main pillars of (...) the AI national strategy. AI readiness assessment was applied across education, entrepreneurship, government, and research and development sectors in Palestine (case of a developing country). In addition, it examined the legal framework and whether it is coping with trending technologies. The results revealed that Palestinians have low awareness of AI. Moreover, AI is barely used across several sectors and the legal framework is not coping with trending technologies. The results helped develop and identify the following five main pillars that Palestine’s AI national strategy should focus on: AI for Government, AI for Development, AI for Capacity Building in the private, public and technical and governmental sectors, AI and Legal Framework, and international Activities. Can we engineer conceptual change? While a positive answer to this question would be exciting news for philosophy, there has been a growing number of pessimistic voices in the literature. This paper resists this trend. Its central aim is to argue not only that conceptual engineering is possible but also that it is not even distinctively hard. In order to achieve this, we will develop a novel approach to conceptual engineering that has two key components. First, it proposes a reorientation (...) of the conceptual engineering project away from fixing conceptual defects and towards bringing about conceptual innovation. Second, it offers a new account of when conceptual engineering is successful in terms of etiological functions. We then turn to the reasons that have motivated various forms of pessimism about conceptual engineering and show that, on our novel approach, none of them stands up to scrutiny. (shrink)

One central debate in recent literature on epistemic normativity concerns the epistemic norm for action. This paper argues that this debate is afflicted by a category mistake: strictly speaking, there is no such thing as an epistemic norm for action. To this effect, I introduce a distinction between epistemic norms and norms with epistemic content; I argue that while it is plausible that norms of the latter type will govern action in general, epistemic norms will only govern actions characteristically associated (...) with delivering epistemic goods. This article develops and defends a full account of the nature and normativity of resistance to evidence, according to which resistance to evidence is an instance of input-level epistemic malfunctioning. At the core of this epistemic normative picture lies the notion of knowledge indicators, as evidential probability increasing facts that one is in a position to know; resistance to evidence is construed as a failure to uptake knowledge indicators.

DNR decisions are frequently made in oncology and hematology care and physicians and nurses may face related ethical dilemmas. Ethics is considered a basic competence in health care and can be understood as a capacity to handle a task that involves an ethical dilemma in an adequate, ethically responsible manner. One model of ethical competence for healthcare staff includes three main aspects: being, doing and knowing, suggesting that ethical competence requires abilities of character, action and knowledge. Ethical competence can be (...) developed through experience, communication and education, and a supportive environment is necessary for maintaining a high ethical competence. The aim of the present study was to investigate how nurses and physicians in oncology and hematology care understand the concept of ethical competence in order to make, or be involved in, DNR decisions and how such skills can be learned and developed. A further aim was to investigate the role of guidelines in relation to the development of ethical competence in DNR decisions. Individual interviews were conducted with fifteen nurses and sixteen physicians. The interviews were analyzed using thematic content analysis. Physicians and nurses in the study reflected on their ethical competence in relation to DNR decisions, on what it should comprise and how it could be developed. The ethical competence described by the respondents related to the concepts being, doing and knowing. In order to make ethically sound DNR decisions in oncology and hematology care, physicians and nurses need to develop appropriate virtues, improve their knowledge of ethical theories and relevant clinical guidelines. Ethical competence also includes the ability to act upon ethical judgements. Continued ethical education and discussions for further development of a common ethical language and a good ethical working climate can improve ethical competence and help nurses and physicians cooperate better with regard to patients in relation to DNR decisions, in their efforts to act in the best interest of the patient. This work is a manifesto for epistemic independence: the independence of good thinking from practical considerations. It presents a functionalist account of the normativity of assertion in conjunction with an integrated view of the normativity of constative speech acts.

ABSTRACTSeveral philosophers have inquired into the metaphysical limits of conceptual engineering: ‘Can we engineer? And if so, to what extent?’. This paper is not concerned with answering these questions. It does concern itself, however, with the limits of conceptual engineering, albeit in a largely unexplored sense: it cares about the normative, rather than about the metaphysical limits thereof. I first defend an optimistic claim: I argue that the ameliorative project has, so far, been too modest; there is little value theoretic (...) reason to restrict the project to remedying deficient representational devices, rather than go on a more ambitious quest: conceptual improvement. That being said, I also identify a limitation to the optimistic claim: I show that the ‘should’ in ameliorative projects suffers from a ‘wrong-kind-of-reasons’ problem. Last but not least, I sketch a proposal of normative constraining meant to address both the above results. The proposal gives primacy to epistemic constraints: accordingly, a concept should be ameliorated only insofar as this does not translate into epistemic loss. (shrink)

Emergency teams facing critical adversity situations often feel questioned in their professional roles as conscientious rescuers, leading to feelings of threats as a kind of stress experience. According to the stress-as-offence-to-self theory, perceptions of insufficiency and disrespect trigger threats by frustrating underlying needs. In this study, we explored threats in the context of a CAS by investigating the activation of threat triggers during the action and postaction phases of teamwork, and evaluating the mediating role of needs. In a multitask experiment, (...) student teams experienced a controllable mission, followed by a CAS mission in a computer simulation task. After the CAS, teams received negative feedback. We measured threats, the activation of insufficiency and disrespect triggers, and the frustration of needs. While insufficiency triggers were activated in the CAS but not in the non-CAS, disrespect triggers were activated by situation-nonspecific and situation-specific feedback but not by no feedback. Furthermore, the results of mediation models indicated the presence of the postulated need-based mechanism between triggers and threats. Our study highlights that the action and postaction phases of a CAS pose a variety of risks for experiencing threats. As individuals cope with these risks, needs are important mediators. (shrink)

This paper has two aims. The first is critical: I identify a set of normative desiderata for accounts of justified belief and I argue that prominent knowledge first views have difficulties meeting them. Second, I argue that my preferred account, knowledge first functionalism, is preferable to its extant competitors on normative grounds. This account takes epistemically justified belief to be belief generated by properly functioning cognitive processes that have generating knowledge as their epistemic function.

When in the business of offering an account of the epistemic normativity of belief, one is faced with the following dilemma: strongly externalist norms fail to account for the intuition of justification in radical deception scenarios, while milder norms are incapable to explain what is epistemically wrong with false beliefs. This paper has two main aims; we first look at one way out of the dilemma, defended by Timothy Williamson and Clayton Littlejohn, and argue that it fails. Second, we identify (...) what we take to be the problematic assumption that underlies their account and offer an alternative way out. We put forth a knowledge-first friendly normative framework for belief which grants justification to radically deceived subjects while at the same time acknowledging that their false beliefs are not epistemically good beliefs. In his early writings, Rădulescu-Motru acknowledged as a valuable gain the fact that psychology had emancipated itself from the tutelage of metaphysics. However, he believed that this progress could not lead to a scientific psychology unless its outcomes were integrated into the broader scientific understanding of nature. In the following, I will discuss Rădulescu-Motru’s ideas concerning the founding of psychology as a natural science. His project essentially involves three directions: (1) conceptual reconsideration, (2) uniformization, and (3) regulation of the soul. (...) Veterinarians are increasingly confronted with new technologies, such as Precision Livestock Farming (PLF), which allows for automated animal monitoring on commercial farms. At the same time, we lack information on how veterinarians, as stakeholders who may play a mediating role in the public debate on livestock farming, perceive the use and the impact of such technologies. This study explores the meaning veterinarians attribute to the application of PLF in the context of public concerns related to pig production. Semi-structured interviews were (...) carried out with pig veterinarians located in the Netherlands and Germany. By using an inductive and semantic approach to reflexive thematic analysis, we developed four main themes from the interview data: (1) the advisory role of the veterinarian, which is characterized by a diverse scope, including advice on PLF, generally positive evaluations and financial dependencies; (2) the delineation of PLF technologies as supporting tools, which are seen as an addition to human animal care; (3) the relationship between veterinarian and farmer, which is context-related, ranging from taking sides with to distancing oneself from farmers; and (4) the distance between agriculture and society, in the context of which PLF has both a mitigating and reinforcing potential. The present findings indicate that veterinarians play an active role in the emerging field of PLF in livestock farming. They are aware of and reflect on competing interests of different groups in society and share positions with different stakeholders. However, the extent to which they are able to mediate between stakeholder groups in practice seems to be constrained by external factors, such as financial dependencies. (shrink)

According to anti‐reductionism in the epistemology of testimony, testimonial entitlement is easy to come by: all you need to do is listen to what you are being told. Say you like anti‐reductionism; one question that you will need to answer is how come testimonial entitlement comes so cheap; after all, people are free to lie.This paper has two aims: first, it looks at the main anti‐reductionist answers to this question and argues that they remain unsatisfactory. Second, it goes on a (...) rescue mission on behalf of anti‐reductionism. I put forth a novel, knowledge‐first anti‐reductionist account, which I dub ‘Testimonial Contractarianism’. According to the view defended here, in virtue of the social contract in play, compliance with the norms governing speech acts is the default position for speakers. Insofar as norm compliance is the default for speakers, I argue, all else equal, entitlement to believe is the default for hearers. (shrink)

To promote youth leadership and more inclusive politics, youth organizations in Nigeria mobilized successfully for a constitutional reform to lower the eligibility age to run for political office. In this paper, we draw on global data to assess whether lower eligibility ages will in fact lead to higher levels of youth participation. We find that lower age requirements positively affect the representation of the youngest and next youngest cohorts in parliament. (...) We draw on qualitative interviews and gender literature to theorize that lower age limits have immediate and longer-term “mobilizing effects”, shifting the calculations of potential candidates in terms of the age at which they first decide to run for office. As clinicians, this case1 raises both personal and professional challenges. A key issue is who carries legal parental responsibility for the difficult decisions that may be required around life-sustaining care in baby T. Medicolegally, we understand that the surrogate mother holds legal parental responsibility for baby T until this can be transferred to the intended parents.2 But this process can take many months to complete, after the birth of baby. As M is now critically ill and unable to engage in (...) any discussion around the care of her baby, who becomes the legal guardian of the baby, for complex decision making that involves either reorientation of care away from intensive care with the inevitable consequence of death in this extremely premature baby, or continuing this life-sustaining treatment with a high likelihood for major neonatal morbidity and longer term disability? Does it pass onto one of the IPs as the genetic father, or does this responsibility fall on the neonatal consultant managing the baby in intensive care? And if an antenatal surrogacy agreement had existed, is it legally binding for inclusion of the IPs perspectives in the decision-making, especially if our medical decision making turns out to be at discord with the beliefs and wishes of the IPs for baby T? One popular view in recent years takes the source of testimonial entitlement to reside in the intrinsically social character of testimonial exchanges. This paper looks at two extant incarnations of this view, what we dub ‘weak’ and ‘modest’ social anti-reductionism, and questions the rationales behind their central claims. Furthermore, we put forth an alternative, strong social anti-reductionist account, and show how it does better than the competition on both theoretical and empirical grounds.

Background:Within oncology and hematology care, patients are sometimes considered to have such a poor prognosis that they can receive a do not resuscitate order from the physician responsible, stipulating that neither basic nor advanced coronary pulmonary rescue be performed in the event of a cardiac arrest. Studies on do not resuscitate decisions within oncology and hematology units, focusing on the specific role of the nurse in relation to these decisions, are scarce.Objective:The aim of this study was to investigate hematology and (...) oncology nurses’ experiences and perceptions of do not resuscitate orders, in order to achieve a deeper understanding of the nurses’ specific role in these decisions.Research design:A qualitative, descriptive methodology with individual semi-structured interviews was used.Participants and research context:A total of 15 nurses from eight hematology/oncology wards in four hospitals in Sweden were interviewed individually.Ethical considerations:In accordance with national regulations, an ethical review was not required for this study. The research followed international guidelines for empirical research, as outlined in the Helsinki Declaration.Findings:The nurses strived for good nursing care through balancing harms and goods and observing integrity and quality of life as important values. Experienced hindrances for good care were unclear and poorly documented decisions, uninformed patients and relatives, and disagreements among the caregivers and family. The nurses expressed a need for an ongoing discussion on do not resuscitate decisions, including all concerned parties.Conclusion:In order to provide good nursing care, nurses need clear and well-documented do not resuscitate orders, and patients and relatives need to be well informed and included in the decisions. Recent literature features an increased interest in the sufficiency claim involved in the knowledge norm of assertion. This paper looks at two prominent objections to KNA-Suff, due to Jessica Brown and Jennifer Lackey, and argues that they miss their target due to value-theoretic inaccuracies. It is argued that the intuitive need for more than knowledge in Brown’s high-stakes contexts does not come from the epistemic norm governing assertion, but from further norms stepping in and raising the bar, and Lackey’s purported (...) quality-driven case against KNA-Suff boils down to a quantitative objection. If that is the case, Lackey’s argument will be vulnerable to the same objections as Brown’s. (shrink)

This article explores the recent urban transformations of downtown Cairo, in particular around the area of Mohammed Mahmud Street and Tahrir Square, after a year and a half of violent confrontations between the protesters and the military junta. The article first looks at how these confrontations led to the segregation of the city through the use of buffer-concrete walls, army tanks, check-points and barbed-wire barricades that made life for its inhabitants impossible. The squeezing of Tahrir and its surroundings created mostly (...) a delineated and restricted war zone. This was undertaken after a series of killings and massacres took place. The article reflects upon the explosion of sardonic graffiti and epic murals that followed these events as a vibrant expression of dissenting street art, as well as the creation of a memorial space. Assertion is the central vehicle for the sharing of knowledge. Whether knowledge is shared successfully often depends on the quality of assertions: good assertions lead to successful knowledge sharing, while bad ones don't. In Sharing Knowledge, Christoph Kelp and Mona Simion investigate the relation between knowledge sharing and assertion, and develop an account of what it is to assert well. More specifically, they argue that the function of assertion is to share knowledge with others. It is this function that (...) supports a central norm of assertion according to which a good assertion is one that has the disposition to generate knowledge in others. The book uses this functionalist approach to motivate further norms of assertion on both the speaker and the hearer side and investigates ramifications of this view for other questions about assertion. This paper develops a novel account of the nature of disinformation that challenges several widely spread theoretical assumptions, such as that disinformation is a species of information, a species of misinformation, essentially false or misleading, essentially intended/aimed/having the function of generating false beliefs in/misleading hearers. The paper defends a view of disinformation as ignorance generating content: on this account, X is disinformation in a context C iff X is a content unit communicated at C that has a disposition to generate (...) ignorance at C in normal conditions. I also offer a taxonomy of disinformation, and a view of what it is for a signal to constitute disinformation for a particular agent in a particular context. The account, if correct, carries high stakes upshots, both theoretically and practically: disinformation tracking will need to go well beyond mere fact checking. (shrink)

According to Jonathan Kvanvig, the practice of taking back one’s assertion when finding out that one has been mistaken or gettiered fails to speak in favour of a knowledge norm of assertion. To support this claim, he introduces a distinction between taking back the content of the assertion, and taking back the speech act itself. This paper argues that Kvanvig’s distinction does not successfully face close speech-act-theoretic scrutiny.
